Declaration of Loss—Cashier's, Certified, or Teller's Check

This document allows a payee or drawer of a cashier's, certified, or teller's check to stop payment on the check (after 90 days from the date of the check) and make a claim for the cost of purchasing that check. Using this form eliminates the need for a bond to be posted to stop payment and make a claim. (In New York, use Form STOP-CC-NY.)
